Privacy: Your privacy is important to me. Your protected health information will not be forwarded to anyone without your express written consent. Your information will not be forwarded to you via email or other electronic media without your written consent and authorization.
Missed appointments: All session recipients will receive an email or phone reminder the business day prior to the appointment. If an appointment needs to be rescheduled it should happen at the time of that reminder or before. If no effort is made to reschedule, a fee will be assessed in the amount of the session fee.
Returned checks: There will be a $25 fee assessed on all returned checks.
